See it all the time at work, the first sign is normally the belt starts squeaking, then you notice the polished part of the pulley where the belt runs is no longer central, then it falls apart!
Inside is just a nylon bush, and if you watch the thing bounce around as the engine is running, you can see why they eventually fail.
